- ✪ ✪ ✪ ✪ ✪Ultimate Health Plan, Medicare Hmo pays back all part B premiums. It pays claims that are properly pre authorized. If the claim is denied, either the provider billed for something different from what was authorized or the provider did not obtain authorization prior to the procedure or service. The patient has to be proactive and the providers know or should know they have to ask the patient's pcp for preauthorization. If a different service is provided, the provider has to ask the pcp to update or correct the authorization. Ultimate has very stringent authorization guidelines making it more difficult and time consuming for the providers but they do pay claims once their guidlines are met.April 14, 2017by J Howland
